Briefly describe the project. - What were your original objectives?
| The town of Chowiefat is a residential and industrial area located at the cost of the Sahel Aley area. This area is highly populated and is directly next to a very busy highway that joins the capital city Beirut to the South of Lebanon, and is a very busy industrial area. The Municipality of Chowiefat, in cooperation with the Lebanese Red-Cross, started a new project to open a Red-Cross center in the Municipality premises itself, where it will be taking on its own account the running cost of the center, and the Lebanese Red-Cross will be in charge of the operations and the staff. The Rotary clubs of Sahel Aley and Los Angeles Cedars were requested to provide this center with its first Medical Ambulance that will be supplied with high-tech equipments. | - What was actually accomplished? | The Red-Cross center was officially opened and operational on June 1st 2008 (with a delay of 2 weeks due to the troubled situation in Lebanon during the month of May). The EMS team of the Red-Cross worked along with the Rotary team to make sure that the ambulance was prepared as required, and made sure that the Red-Cross sign was there and so was the Rotary donation emblem. | - When and where did the project take place, and who were the beneficiaries?
| The Chowiefat Municipality held a big ceremony where around 120 people from the surrounding community, head of municipalities, NGOs and the press were invited. District 2450 governor (Farid Gebran) was invited too. Speeches were said from all three parties (municipality, Red-Cross & Rotary) that illustrated highly on the spirit and enthusiasm that Rotarians have to serve their community. The ambulance will be used by this center to serve all in need in the costal area and its surroundings. | 2. Scope change. How did Rotarians manage and oversee the project? | The Rotarians provided the follow-up and coordination between the suppliers, the Red-Cross and the municipality. The Host Rotarian partners kept the international partners updated during and after the project fulfillment. | 4. How many Rotarians from the host partner club participated in the project? | 6 | 5. Please list all non-financial involvement. | The Host Rotarians assigned a committee of 3 members that kept in direct coordination and facilitated the communication during the life of the project with a team assigned by the Municipality and the Red-Cross, and the Suppliers. What was the impact of the project on the beneficiaries? | The red-cross ambulance is to be used by the red-cross center to help transfer sick or badly injured people from-to hospital. It is also used by the center to help in transferring people from car/work/home accidents to the nearest hospitals. | 10. If there is more than US$200 remaining, it must be returned to The Rotary Foundation. [Note: In India, government rules require that all unutilized funds be returned to the Rotary International South Asia Office.] - For grant awards over US$25,000, attach an Independent Financial Review to each progress report and the final report.
- Keep all original receipts for at least five years, or longer if required by local law. Do not send receipts to TRF unless requested by staff.

14. Certifying Signature – Either the Host or International Partner must certify the report. If the grant is club sponsored the current club president must certify the report and if the grant is district-sponsored the district grants subcommittee chair must certify the report. By signing this report, I confirm to the best of my knowledge that these Matching Grant funds were spent according to Trustee-approved guidelines and that all of the information contained herein is true and accurate. Original receipts for all expenses incurred will be kept on file for at least three years, or longer if required by local law, in case they are needed for auditing purposes. I also understand that all photographs submitted in connection with this report will become the property of RI and will not be returned. I warrant that I own all rights in the photographs, including copyright, and hereby grant RI and TRF a royalty free irrevocable license to use the photographs now or at any time in the future, throughout the world in any manner it so chooses and in any medium now known or later developed.
